INSURANCE INFORMATION

We recognize that the cost of dental treatment can be overwhelming for our patients and so we submit our bill for the dental services directly to the insurance company.

We accept most insurance plans including but not limited to: Cigna, Guardian, Delta, Aetna, United Healthcare, and Anthem.

We are members of CIGNA, Aetna & Guardian DMO. These plans have patient co-pays required at the time of service.

In addition, we participate in discounted fee plans such as CAREINGTON, Signature and AmeriPlan. Please remember that co-payments are required at the time of the visit.

Our office accepts MasterCard, Visa, Discover, American Express.
We also offer a medical credit card called CareCredit which allows our patients to have the dental work they need or the cosmetic changes they always dreamed of by extending their payments up to 4 years!
As a courtesy to all of our patients who do not have dental insurance we extend a 10% discount on our fees.

Most insurance plans require us to keep a signed, hard copy of an insurance form in your chart. For your convenience you can complete the universal insurance form, download it and bring it with you at your initial visit. Or we will be glad to assist you in filling out the form when you arrive at our office.
